Getting started
Start by working through the set-up phase of this project, setting yourself a goal according to the time you have available to dedicate to this project, ensuring your team are aware of the project and you have been given the authorisation from your practice manager to complete the project.

The aim of this Quality Improvement Project is to ensure 50% or more of people on the Asthma register (both adults and children), have a personalised Asthma Action Plan, and evidence this with the appropriate SNOMED code in their notes by November 2021.
This is the second of two projects, which involves reviewing a patient’s Personalised Asthma Action Plan during their next Asthma Review. This might be using the traditional paper copy of the PAAP, or the patient might be using the NHS Wales Asthmahub or Asthmahub for Parents apps.
This QI project is aimed at primary care teams who have a register of Asthma patients (adult and paediatric).
Data collected from GP practices across Wales suggest that only 16.1% 1-5 year olds, 24.3% 6-18 years olds, and 25.9% adults have any record of a personalised Asthma Action Plan in their records.
This QI project will ensure that every patient on the Asthma register has access to a Personalised Asthma Action Plan.
Good data entry directly improves patient care, which will also help you identify gaps or errors in the patient record or diagnosis.
On completion of this QI project, you will also get a certificate for your work. You will also be able to use this QI project as evidence towards CPD and professional revalidation.

Capacity is the most important aspect for the successful completion of any project – ensure you have the capacity, time and ability to complete your project in a suitable timeframe.
Start by outlining how much time you have to commit to this project.

Step 1 – During a patient review
During a routine Asthma review, identify any patients who do not have a Personalised Asthma Action Plan (PAAP), and ensure they have an up-to-date Personalised Asthma Action Plan and the patient knows how to use it.
Step 2 – Code the PAAP
Code the patient’s Personalised Asthma Action Plan in their notes, using the following codes:
